Visa information

If you need to obtain a visa to enter the U.S., the Southwest Scoliosis and Spine Institute can help. After your treatment plan and appointments are verified, we will write a letter on your behalf to help you obtain your visa. Accordingly, you must bring this letter with you to a U.S. embassy or consulate when you apply for a visa.

In case you need to extend your stay for your treatment, we recommend that you obtain a B1 (business) or B2 (pleasure) visa. Further information about online visa submission is available on the US Visas website.

Your medical records and forms

When you schedule your first appointment with a Southwest Scoliosis and Spine Institute physician, please confirm the records and forms you will need to bring. If you do not have them, your treatment may be delayed. In most cases, you will need:

  • A hard copy of all medical reports translated into English.
  • Diagnostic imaging results. Bring the films and/or CD ROMs of any diagnostic testing relevant to your medical condition.

  • The DFW International Airport is 20 miles from the Southwest Scoliosis and Spine Institute Dallas Offices.
